How do I get started with SoundChoice?
Applying for SoundChoice is as quick and easy as 1, 2, 3:

If your hearing healthcare office offers SoundChoice:
Simply complete the short application provided by your healthcare professional. A staff member will submit your application and you will know within minutes if you qualify. Once qualified, you can purchase your hearing instruments immediately.

If your provider doesn’t offer SoundChoice:
Call 800-328-8602, ext. 2491 to locate an office that accepts SoundChoice in your area or refer your hearing professional by clicking on the Locate a Hearing Care Professional link on the top of this page.

Apply online yourself:
Click here to use SoundChoice/CareCredit secure online application and you will receive a decision in seconds! Simply fill out the application, submit the app, and wait online for your decision to appear. Once you are approved, print out your response and present it, along with two forms of identification, to your hearing healthcare provider.

You will receive a SoundChoice / CareCredit credit card in the mail, which you can use at providers throughout the country, including dental, ophthalmology, veterinary, audiology, chiropractic, hair restoration, and other healthcare practices nationwide.
